Where the questions in this series come from

From consulting work, not from the literature. Who owns a decision once a system has prepared it? Who answers for how that system is configured? And what does it cost when neither has been settled?

I asked these questions in projects first, and found how rarely they have an answer. Not because nobody is responsible — but because the responsibility sits in a different place from the consequences.
